Token size
What determines the size of the token when you drag it from a token bag to a grid? Is the size set by monster size (combat tracker or npc) or is there sometching in the token itself that sets the size?

There's no information held in the token. Depending on the ruleset, if token auto-sizing is set in the campaign options, the size of the NPC the token is attached to in the combat tracker can impact how many squares the token will take up; otherwise it will be sized to one square.

If you just drag a token from a bag to an image without a grid it will retain its own dimensions.
If you just drag a token from a bag to an image with a grid it will use the scaling options set for your campaign.
