Confused about Initiative

Do the players and NPC roll for Initiative, or is it just always based on the Number on their sheet? And if they do roll for it, how do they?

There is no rolling for initiative from the core rulebook.

You can see your combat options in chapter 2 of the reference manual. The core rulebook was setup as an almost one for one copy of the core rulebook.
