Is it possible to code a NPC barbarian with Rage in their Stat Block...
Or do I need to manually add the effects in the Combat Tracker?

Manually add the effect or you could use this extension which may do what you want. https://www.fantasygrounds.com/forum...cs-characters)
If there is something that you would like to see in Fantasy Grounds that isn't currently part of the software or if there is something you think would improve a ruleset then add your idea here https://www.fantasygrounds.com/featu...rerequests.php

I have always wondered..... what is the deciding factor whether something gets added to the ruleset, or they leave it for others to create (and charge more money for)?

See here for the kinds of things which the parser picks up for NPCs and creates the effects for. https://fantasygroundsunity.atlassia...and+Encounters
