What about the DM that switches it off when PCs are exploring but switches it on when in combat. Whatever way you do it it will require some switching on and off; so it doesn't matter if it's on or off by default - many DMs will need to intervene one way or another. From what I can gather the majority have it on so having it on by default seems to suit the majority I'm thinking.

I am definitely in the default off camp. At the very least is should be a global option so it doesn't have to be done on *every single map*

I'm always a little surprised when I hear GMs talking about players running around uncovering the map. This seems like the VTT equivalent of them peeking behind the dungeon master's screen and looking at his notes. Why tolerate this behavior? If someone couldn't control themselves after a warning, I'm not sure I'd want to game with them.
