Can you confirm you have the authors permission to share this module? Even though the PDF is free it doesnt mean that you can change its format etc unless it has a license saying you can or you have the authors permission.

I had it on Lulu also but Lulu got really weird when they updated it. So Amazon is where it will stay for sale.
