I updated the strain and injury extension.
These are the modules and extensions created and/or taken over by dellanx for PFRPG.

dellanx, if you want help you have to provide more detail. What ruleset are you running? Deactivate all extensions and see if it occurs. Start up a fresh campaign and make sure it doesn't occur. Not even God could help you with the lack of information in your post.
