Just clarifying what the above are saying, but you can often press down and hold your mousewheel. It is a third button.

Ah, yes, I now understand what damned was saying. Thanks! Yes indeed, this does work. Of course, I happen to have one of those mice where the third wheel tilts to do something specific so this because a bit of a nuisance...but it definitely works! I would still enjoy and combo keyboard+left-click+drag solution since that would help those without mousewheels or third buttons. On occasion I play while traveling and I am limited to just my right/left buttons on the keyboard or a limited function travel mouse.

Some laptops allow for a "middle-button" by holding down both laptop button points at the same time.

I would also greatly like to be able to lock the window position of the image window, and have a toggle I can click that swaps the left-click and drag from moving the whole window to panning the image. That would be wonderful.
I never claimed to be sane. Besides, it's more fun this way.
