Weapon creation fail

Some of my players have lost weapons in their action tab. They re-equipped them from the inventory and they didn't populate in the action tab. They can't create a new weapon and I can't create one for them.

Make sure that the Mode in the bottom of the actions tab is set to "Preparation" - this will show all of the weapons they have. Then click the icon to the left of the weapon names until it shows a chain shirt symbol - this equips the weapon and it will be visible in all modes.
