I'm probably missing the obvious, but how does a player (yes, I have the full version) set an avatar for the sheet they upload to the game? I found a great image, but while I can see the avatar, I can't seem to attach it to the sheet.

By Avatar, I am assuming you mean the character portraits in the top left of the FG desktop. It is assumed by FG that the GM doesn't have any need to add a portrait to PC characters. Portrait images come from the player/client computer and are transfered to the host/server normally.
Probably the easiest way to add a portrait to a server character is to start a second instance of FG on the same machine and log in with a different log in ID as a player. You can then select a portrait on the player instance with the portrait button in the top right. This is how you might also set up pregen characters with portraits for players.

Are you talking about Portraits or Tokens?
PC's have portraits. These show up on the character selection screen when you first connect. They are set by clicking on one of the buttons in the upper right hand corner of the FGII screen (in the Module Activation, Characters, Options, Combat Tracker area). Portraits are what are displayed in the top left of the screen when players are connected. Optionally they can be displayed on chat messages. NPCs do not have portraits.
Both NPCs and PCs have tokens. Tokens are how the PC and NPCs are represented on maps. There is usually a field on the PC's character sheet or on the NPC personality sheet where the host can drag tokens from the token box and associate them with the PC/NPC. Optionally PC's can drag their Portrait to the spot on their character sheet and use this as a token.

I think I may have been asking about both at first, but I suppose it's Tokens for the monsters then. All I see in the token box are three little "stamps" for want of a better description. They all look exactly the same.

The DM has to provide his own tokens. You can use any image you want for them, but it's easiest to have them all the same size, that saves resizing once you put them on a map (you can resize with the scrollwheel on the image in the combat tracker). I forget where tokens go, but once you get to the Application data folder it's pretty self-explanatory. (I advise putting the tokens in the "Host" part so that the players cannot see what is ahead of them, unless you want to give them a wrong idea >:) )

Look in the little "bags" in the token box. One of them should have a token for each letter. As Zabulus mentioned you can add your own tokens in FG's data app folder. There is a tokens folder with two sub directories: shared and host. Tokens placed in shared are shared with the players at the start of each session and tokens put in the host folder are only shared when they are placed on the map.
You can make your own "bags" by making folders under Shared or Host. I suggest this. For example in my tokens\host folder I have several folders: Generic Monsters, Monsters, NPCs, Walls. Inside the Monsters folder I have another set of folders: A, B, C, ect and I sort my monster tokens alphabetically. Note: you have about 25 slots in each bag with no overlap and about 50 with a small overlap (depending on the size of your tokens - I'm refering to <= 50 pixel tokens).
Token folders in the host and shared folders that have the same name are combined on the host.

Yeah, I'm using the 4th edition as well. Forgot to mention that.
