I believe it's an image with a grid set on it so conditions can be monitored (or something like that). I think it was a workaround for some missing functionalilty in the ruleset or may be just down to tokens not stacking properly for marking targets.

I'm going to GameCon08 and should be getting to play my first 4ed game in the Living Forgotten Realms sessions they are having. So looking forward to it, have booked in for a Savage Worlds game as well :)
