Global Portrait folder vs Campaign Portrait Folder

What do we use the campaign portrait folder for? None of my player portraits appear in the FG when I use the campaign folder, I have to put them in the general portraits folder.

That is correct the top level portraits are for you when you are a player. The other one is for the players in your campaign. When your players connect and select a portrait the portrait is transfered to the server and the server resends the portrait to the other players. The server stores this portrait in the campaign portraits folder under the name of the character without any extention. Until you have players create a character and set a portrait this folder will stay empty.
