FGU automatically triggers concentration saving throws when a character is damaged while concentrating. However, when a character that is concentrating is Incapacitated (other conditions that include Incapacitated are: Paralyzed, Petrified, Stunned, and Unconscious) the concentration should end automatically.
I'm not sure if this is an oversight or FGU limitation, but I'm hoping to find a solution so that I don't have to manually go into each character when these condition effects are applied and double-check for any concentration effects to remove manually during play. Thoughts?

This is working for me. Can you give a specific instance where it's not working? And also make sure you are testing without extensions since one (or more) might be interfering if you are using any.

Oh, interesting. If I use the Incapacitated button, it works appropriately as you've shown here, Zacchaeus. What I'm seeing is that if you use the green + in the CT to add an effect and type in Incapacitated, it doesn't work. So, it's just that when you manually add an effect it isn't executing properly.

Indeed. Manually typing in an effect isn't going to have an impact. The effect would need to be applied either from the effects dialog or from another actor.
