Recently, when a PC rolls a natural 1 on a skill check (with a DC set via the Request Rolls extension), it is incorrectly reported as a success.

Even with double nat 1s on a skill check with disadvantage:

65869

I don't think it's related to the extension (unless it was a recently introduced issue). Has anyone else seen this?

It's consistent (but takes a while to duplicate). It happens for different PCs and different skills.

Is there some option or setting that could affect this behavior?

A natural 1 on an ability check is not an automatic failure like it is on an attack roll. The modifiers are added and compared to the DC. In your screenshot, the net roll after mods is an 11 vs a DC of 3, resulting in a success.

What Cyrian said is correct.
A natural 1 is not automatically a failure, you still have to add all the bonuses to the roll and then compare it to the DC.
A natural 1 is an "automatic miss" only for attack rolls, nothing else. Even when rolling a saving throw, a natural 1 can still be a success if the character has a high enough bonus to the roll.
This is true in D&D 5e both in 2014 rules and 2024 rules.

Natural 1s and 20s only matter for attack rolls and death saves. 1s can pass anything else, and 20s can fail anything else.

It's worth noting that while guidance is that a roll should only be called for if the outcome is at least theoretically in doubt, it is also perfectly reasonable to call for a roll where a 1 may be enough or a 20 insufficient. Discretionary and randomized modifiers exist, like maybe that 1 would have failed if Bane were in effect and a maximum penalty of 4 had been applied; while a 20 that might not by itself suffice, could be enough if high rolls were scored on Bardic Inspiration, Guidance, superiority dice, or similar additional stuff.
