New 5e campaign, no extensions, only LMOP and legacy PHB and MM loaded.
GM side - Share an image to clients - the indicators (tiny portrait in bottom left of image) for which clients currently have the image open are missing.
(Noticed this yesterday with an old/huge campaign with tons of extensions, but they worked sometimes on some images. Not sure of the rhyme/reason.
Additionally, when they were showing, the GM side did not update when the client switched their active character. Reloading the image on GM side would update this)

I am seeing the same thing (or not seeing the portraits) and mentioned it to my players. It is a small thing, but was helpful to make sure everyone was viewing the map. 5E Dungeon of the Mad Mage.

Honestly, I think this is an extremely useful feature. Having to constantly ask if people can see a shared image is brutal... and assuming they did when they didn't is possibly worse.
I'd love to see this work on every image every time.

Just pushed a fix to restore this capability. Please run a new Check for Updates.

This is currently good enough (and way better than it not working or working inconsistently) from what I can tell... but did it ever use to swap portraits on the GM side when a player changed which PC was their active PC?
Currently:
GM shares a previously unshared image to a client who is logged in with PC A and PC B. PC A is currently selected on client (so they their dice/color shows, etc.)
GM sees PC A portrait (as expected)
Client changes to PC B
GM still sees PC A portrait
GM closes image
GM reopens image
GM now sees PC B portrait
I would say in a perfect world, the GM should see the current one, whichever it is without reopening the image:

GM sees PC A portrait (as expected)
Client changes to PC B
GM immediately sees PC B portrait

When you share an image you share it with the client not the character. So whichever character is selected at the time the image is shared will show in the teeny tiny window at the bottom. (Not even sure how you manage to make out what portrait is showing - I just count the dots).

It does also update that portrait based on the client's currently selected character. It just doesn't doesn't push that change to the GM side... it does show the new portrait if the GM closes/reopens the image.
I don't think this is a major issue or a high priority, but if I had the option between the two, I'd rather it updated instantly on the GM side and don't see any downside to it working that way (except obviously development time).
Regarding seeing what the portrait is showing. I am fortunate to be playing on a 32" 4k display (at /scaleui 160%) so it is actually pretty easy to make out the details. In a perfect world, these portraits would be resizable/moveable (like I wish essentially everything was... but I dream).
