LOS not working on a single map image

I was adding lighting to one of my maps for an upcoming session and for some reason the LOS and lighting are not working. I can add a token to any other map image and everything is fine but this single map isn't working.

I have LOS toggled on and Lighting toggled on but not sure what the issue could be.

I imported the same image again and tried the LOS on the new image, and it works fine... I then went back to the old version of the image and it's working now... Ghosts in the machine?
