Errors with overlapping tokens

This weekend we had a strange situation, the group was fighting a shambling mound that had grappled one of the players. I moved the token onto one of the squares to signify it was grappled. The paladin charged into the square beside it and tried to attack but we got a slew of errors. Moved the token off the other one and the errors persisted. When we moved the token two squares away, the paladin could attack the SM with no errors. Meanwhile, everyone else in the group could base the SM and attack it regardless of the position of the grappled PC. Very strange. The picture of the red bearded dwarf is the one that was grappled and this was us trying to get the error not to show.

The vast majority of these type of errors are caused by extensions. The error listed is from a custom attack handler. The full name of the extension is truncated, but it could be this: https://forge.fantasygrounds.com/shop/items/406/view

I figured it was from an extension, just strange that no one else suffered any errors. Unless something specific in that characters attack was causing it.
