BAB issue was likely user error and impatience. The UI glitch is still unknown as it appeared to happen to a completely random player both times I witnessed it occur. going to leave the rest of my post as is in case any other new players/GMs find this issue and can use my BAB mistake to learn from or also report the odd UI glitch

Summary:
I am running Pathfinder 1E and my players have just reached level 6 with the paladin taking a prestige class into knight of Ozem, BAB has hit 6 but the sheet refuses to allow for any form of extra attack and has them stuck at 1. I at first thought perhaps my modules or extensions were an issue but I loaded in with everything switched off and it remained, I then tested it on a seperate character sheet and they also only had one attack at a BAB of 6. Withnin the same session a single player became unable to use their character sheet easily due to the UI being slightly off centre and the buttons registering about 1/2 cm below their position, the tokens on the map were also off centre for this player. This bug I witnessed happen in a session I was a player in a day ago also with that being a DnD 5e game. Token targeting regarding this player even for me as GM was a little difficult as his token didn't register properly on the map until randomly the issue resolved itself part way through the game.

Ruleset(s):
Pathfinder 1E

Extensions/Themes:
Hearth Theme, every bmos extension

Modules Loaded:
PFRPG-Spellbook Extended, PFRPG-Races, PFRPG-Feats, Turboguy's classes and archetypes mod

Operating System / Language Setting:
Windows 11 English

Steps to Reproduce:

I'm not completely certain how to reproduce the off centre UI phenomenon as it seems random but the BAB bug seems to be repeatable on multiple character sheets, the issue does not seem to effect statblocks just PC character sheet nor does it seem to be reliant on extensions, modules or theme

This appears to be working correctly without any community extensions. (see video)

You'll need to figure out which extension is causing the issue, and report it to the community extension developer. The issue does not appear to be in the base code/theme provided by our company.

Extensions can leave "residue" in the campaign database. To test you need to reproduce in a new campaign with new characters with no extensions.

As you can see from Moon Wizard's video in post #2 the control of the additional attacks is manual, the display of the additional attack fields isn't linked to the BAB of the PC.

What I was having trouble with was the ability to edit that section to make it 2 attacks was seemingly locked. My assumption was maybe it was locked until BAB 6 was reached and yet it still remained locked.

Though I've loaded in today and haven't had an issue so perhaps I just hadn't waited long enough for fantasy grounds to register I clicked on the box.

It seems my BAB issue is likely user error and impatience on my end I apologise. I'm still unsure about the odd UI glitch though as that has occured in other games not just the one I've run with extensions. I don't yet know what is needed to recreate that issue however and I have not been one of the players effected by it.
