Is there a way to take the character sheet from one system and use it in another ruleset? For example the design of the D&D 5E sheet in the AD&D 2E ruleset. Such as rhis:

That will depend on a lot of things. How good you are at graphic design, how well you can trace back controls to their templates to their graphics, how much time you have etc.
