Whenever I or any of my players make an attack roll (5e ruleset), it opens three maps, two of which are in the same image category, but the other isn't. The third also has other images in its category that don't open. I have no idea what could be causing this issue, if anyone has experienced this and fixed it, or has any insight into why it happens and how to fix it, I'd really appreciate the help!

Welcome to the FG forums. Sorry you're having issues.
I'm guessing that this could be caused by extensions you're running. Disable extensions one-by-one and try to recreate the issue - this should help to identify which extension might be causing the issue.

Does this happen with all extensions disabled? Does this happen in a new campaign? How are the attack rolls being made? (from the hot bar, the actions tab of character sheet, from the combat tracker, or some other way?) Can you record a video of what is happening?

I have this happen when I cycle the CT active person through an entity that is on a different map - it opens that map. For example I have a PC that stays behind to guard the camp, but he's still in the CT. When his initiative comes up, the map with the token on it is opened. Could this be what you're seeing?

Just ran some test with one of my players, and we figured out the culprit, but I'll post a video and details anyway for anyone else who runs into this issue.
He made a melee attack and it was fine, which made us realize it only opened the maps on ranged attacks. The extension that caused the problem was rob2e's Automatic Flanking and Ranged extension. Probably because I don't have the most recent version, but it might not be supported any more, not sure yet. Thank you for the quick replies!

It is supported. Have you reported to robs discord channel.

I have had this happen too. I have noticed it happens when I manually move a token from one map to another instead of deleting the token from the old map and then putting the token on the new map from the CT.
Edit: The only fix I have found after doing that is to either use the remove all tokens radial dial option on the old map or restart FG.
