Combat Tracker Targeting Errior

I am getting an error message trying to target PC characters from my NPC or Monster in the tracker.
Error reads as follows: Unable to target token not linked to the combat tracker entry

What has happened to my dungeon monsters that they are unable to target the PCs?

As the error suggests the token that you are trying to target isn't on the CT; or the link between the token and the CT has become unlinked. Rtry removing the PC from the CT and re-adding them. If you are using extensions try in a new campaign without extension and if the error doesn't happen then it is probably an extension that is causing the issue. You'll need to figure out which one by removing and adding them back in one at a time until you hit the problem.
