It says you will only defend yourself if attacked. What if you are attacked by a missile? Does it "break" the spell?

The target of the spell would likely "take cover" if they could but I expect they'd fight back if cornered.

:pYou betcha.

One thing to remember as a GM with the Calm spell, is that the affected can run away and still act normal in all other respects.
A Calm’d individual still doesn’t wanna see invaders romp around their demesne and will still sound the alarm to ensure the interlopers are caught. It’s just that particular individual turns into a Pacifist for a few minutes in their life, LOL.

For a laugh, Ardem(of the awesome RMFRP FG extension fame) has them do jumping jacks in front of shooters to spoil shots since the so-called “active shooter” now doesn’t want to ofherwise break the spell by shooting the sucker encroaching on his space. Now they have to contend with Full or Half Soft cover because of Jumping Jack Flash!
Also, the bastard(loving term, I swear! ;) ) has also been known to have another nearby NPC of his SMACK the ensorcelled one, thus killing the spell because “it doesn’t say from whom one has to be attacked by…”
Too funny, but it is RAW!
