What does the error say? Whenever you get an error please record the details of it. This makes it much easier to track down.

You can use the Compile Logs feature in the console window to quickly provide all the needed information to investigate whenever you see an error.

I am not getting any errors when deleting NPCs from the Combat Tracker and I used it for 2 sessions before it was released and had to delete 10 different NPCs during those sessions. I also just tested it and can't replicate the issue so as Trenloe mentioned having the exact error will help me track it down better. Also have you tried it without any extensions to see if you can replicate the issue?
