You most likely have an extension that needs to be update to work with the latest version of FGU. Unload extensions and see if the problem is fixed.

The file scripts/manager_action_attack_ptact.lua is not in the 5E ruleset.
You need to update all your extensions. If it still happens unload all the extensions and add them back in one by one until you find the issue.

the name ptact might give you a clue.

It's the Kent McCullough Pack Tactics extension that is giving me fits.
