What would be a good way to create a mod for this?

I think I would just create them as items, and then add them to the actions tab as well so you can track their uses. I don't think you are going to be able to automate their effects without an extension. Though with an extension you would be able to add in a new field to track them. I would lok at the current hero point extensions as a starting place.
