equipped weapons not showing on action tab

Hello,
I am having trouble with some of my players characters are unable to equip a weapon from the inventory page and have it show up on the action tab. Some of the characters still work fine, but then others are unable to equip anything. I did unload all my extensions and it still happened. Could this have been caused by the update yesterday? I am doing DnD 5e ruleset.

I dont think there were any CoreRPG or 5E updates yesterday.
Make sure that there are no unpacked CoreRPG or 5E folders in your \rulesets folder and then rerun the update. If it persists have the affected player do the same and retest.

okay reloading the update now allows a equipped item to show up on teh action tab. However unequipping does not remove it.

Are you in Preparation mode on the Actions tab?

If in Preparation mode, then any weapon entries will always show with an equipped state icon.
If in Combat/Standard mode, then any weapon entries will not show equipped state; and only weapons marked as "equipped" will be shown.

I dont believe that unequipping a weapon does remove the action...
Discarding (deleting) the weapon does remove the attack.

okay that seems to have it. Thanks Moon wizard. I was ion the wrong mode. Combat/Standard mode fixed the problem. Just for future reference how do I pull my logs?
