Add spell action not working

Hi,

I'm trying to add spell action for some spells but it's not working every time? Sometime when I right click to add spell action it's only add a D20 and when a click on the magnifying glass the option I ask to add are missing and I'm not able to delete this D20 from the spell. Can you help me?

To best provide assistance in creating tickets for issues, we need more information than you are providing. Also it helps to provide that information in an established format. The one key question I have is what ruleset is this trying to be performed? There are many D20 systems currently and many of them can create spell actions. If you can please read or re-read the above post and provide your report in the format provided.
