Open-ended RM dice roll

I am sure someone has answered this but I cannot find the answer.
How do you get a d100 open-ended low on 5 or less and open-ended high on 96 or more, a la Rolemaster?
I can get it to do one or the other.

Our group are used to having RM Skills with a AD&D magic system so this would be really handy.

Take a look at the MoreCore Ruleset. I believe it has RoleMaster dice available in it. You could probably pull out the code for those dice and put it into an extension for 5E.
