Did I make these correctly?

I'm trying to make two items for my campaign. One is called Soldier's Respite the other is Soldier's Aid. They are military/city watch issue mini potions. These particular ones were gifted to a treasured son on his acceptance to the City Watch.
Soldiers Respite is a simple healing option that heals 4 points of damage. Pretty sure I've got that one correct.

Soldiers Aid gives the imbiber +2 to damage and to hit for 10 rounds. I think I've got it right.
Could someone in the know look at the pic and let me know if they think I've got it right?

What ruleset? Most rulesets do not automatically create effects when used or as actions unless you are using a community extension.
