Token/Map issue

Hey all. Ran into a new problem... We have been playing a couple different campaigns and today my brother started running his. We placed tokens on the map per normal and moved them where we wanted them and for some reason the tokens themselves stayed in place but a green square appeared where we moved the tokens too. The green square shows the characters names when holding the mouse over it but it is making things very very confusing. Every other time we have played the tokens have moved and re-positioned with no issues... Not sure what could be the problem

Perfect thanks. I tried to explain to my brother that we always use 50 pixel grids and his was set to 7 pixels or something like that. I was afraid the issue was the fact that we were using himatchi but after increasing grid size everything works great. He just needs to redo some maps
