Combat tracker trouble

Is anyone else experiencing any problems with the combat tracker? We have been experiencing problems with the fact that damage will be assigned to characters randomly. We have tried targeting them on the map and in the tracker. The Tracker reads Target A is selected but Target B receives the damage. Sometimes this happens ... sometimes it doesn't. One time tonight one player while targeting an NPC did damage to his character. All the characters are pulled from the combat tracker onto the map. We have been playing for three years now and this is starting to occur on a regular basis.

What ruleset?

Are you running any extensions?

When it happens again please take screenshots of the setup - CT, chat window, etc..

Your issue is being caused by an extension I am pretty sure. I can’t remember which one now, but i’ve seen this come up before.
