I've done a search around but haven't been able to find anything, so let's just throw this out there I suppose! Has anyone made, tried to make, or thought about making a small extension to implement the DMG's Massive Damage rules? Something that would check if damage done is greater than or equal to half the creature's maximum HP, does a DC 15 Constitution saving throw if it is, then applies a system shock effect as given on the DMG table?
