Adding a tab to the 5e character sheet

Can I add a tab to the 5e charsheet with an extension or do I have to actually modify the ruleset? If I modify the ruleset, would that stop the automation via PHB, DMG etc?

All changes to the ruleset should be done via extension.
Over time some extensions become incorporated into the ruleset.
