Player breaks from combat tracker

So we are having an ongoing situation with a player. It seems like he constantly breaks. Meaning when he targets things on combat tracker and rolls, even though it says he has them targeted... it doesn't roll vs that token/sheet and just rolls in chat tied to nothing. However, I can target him and have things work just fine. If he closes FG and reopens, he's back in action but eventually breaks again.

Does anyone have any idea why this happens and is happening to him more and more frequently?

When it happens ask him to click the little target icon in the modifier box under the chat window and try again. This toggle targeting on and off. I'm wondering if they've accidentally clicked it. A restart of FG resets this targeting toggle, so what you describe does suggest the player has accidentally set the toggle to off sometime during the game.
