The idea is basically this: there are certain creatures in the the MM that are examples on merging a template to a creature. Examples include the half-dragon template and the shadow dragon template. Also included would be all the humanoid "templates" in the back of the MM, such as bandit or guard. Those can be used as templates to merge with any humanoid race.
Other examples include the lycanthropes and vampires. The ones listed in the MM are all based on humans. These should also be considered templates if you want an elven lycanthrope, or even a dragon lycanthrope (weredragon).
All these can be handled exactly like the magic item forge.
P.S. You could include my favorite: a vampiric were-dracolich. This is the ultimate in template merging.
