So I decided to port it on my campaign and created a module for fantasy grounds. The original authors must be credited for it. I just ported it in fantasy grounds and applied minor changes to the tables.

Very nice, but I'm going to have to remove this for the time being. Can you expunge the D&D log which is copyright WotC and cannot therefore be used like this. Additionally I cannot see anywhere that you are specifically acknowledging the original authors and that you have permission to distribute it. I also have some trouble with the artwork. Do you have permission to distribute and/or use that? Again an acknowledgement and details of the use should be included. I'm also thinking that you might also have to include the OGL somewhere. I think perhaps the DMsGuild might be a better place from which to distribute this rather than here.
