How do you guys deal with it? Mainly theater of the mind?

The way we ran it in one of my campaigns was to create a second PC sheet (and actually more in later levels) that were modified for the shape. This sheet had it's own token etc and was added to the combat tracker. We would then just use the appropriate token (and therefore CT entry) as needed on the map.

Druids in my campaigns generally set up effects for each form they plan to take, adding temporary hp, modifying their ability scores, and adding the beast's attacks. FG has a very powerful effects engine - you can find some of the effects on other threads and forums on this site.
