5e custom effects

I have a player that is trying to make a custom effect in which a person after a certain spell is cast on them, the character automatically rolls an extra d4 on attack rolls or on wisdom saves. He has been able to get the attack roll to work but not the wisdom save. Can anybody help? Thank you.
