Okay I'm certain I've done something wrong here, but haven't been able to figure out the fix. Yet.

When I start my Lost Mines of Phandelver campaign, and connect to it as a client, I find that the client has access to a single (random?) token on the server. The token comes from the DD MM Monster Manual bag (and it's the W.png file).

I've cleared the cache, but that's not the solution. How do I clear this on my server so that it sends out a clean campaign to clients?

have a look in your shared token directories and see if the token is present there. it should be safe to remove it.

Have you previously shared an NPC record with one or more players? If so, and that NPC record had a token linked, then that token would be shared with the players and remain shared. I'm guessing a creature beginning with W? Go to the NPC list and see if there is a (s) or (p) icon after any of the entries. If so, click on hat icon to stop sharing - if you want to stop sharing of course.
