If I select advantage then pick up and roll a single d20 into the chat window, I get the following script error:

Script Error: [string "desktop/scripts/modifierstack.lua"]:244: attempt to index field 'targeting' (a nil value)

This happens on a clean campaign, with no extensions loaded. I'm running 3.0.12, on a Mac in crossover.

Can you see the little targeting icon on the modifier box below the chat window? If no, then sounds like you might be running an old version of the 5E ruleset or a theme.

I don't see any targeting icon. The chat window reports CoreRPG at 3.0.12 and 5e at 3.0.8

I've updated and and update is not shown as outstanding.

That's the issue. You've got an old version of 5E. Open up your <FG App data>\rulesets directory and look for a B]5E[/B] directory, if there is one there either delete it or rename it to 5E_308 or something similar. Then try launching your campaign and checking the version - it should use the latest updated 5E.pak file and show version 3.0.12 in the chat window.

Sorted! :)
