If the ACTN flag is set on an effect on a PC/NPC who has say 3 attacks. All 3 of those atks have the effect applied to them (if applicable). Thats fine.

If the ACTN flag is set on an effect on a PC/NPC thats is a TARGET of the 3 atks (say AC: -4) Then the effect disappears after the first attack from the attacker like a ROLL set flag

Should the TARGET not get all 3 of the attacker's attacks with the effect remaining on the TARGET?

Hard to say without going into the code and having a look. But possibly could be the effect stays on until the turn counter increments.(IE that PC/NPC's turn has finished). But Im only guessing

I'm pretty sure that the "Action" setting will only work for source actor effects, not target actor effects (such as AC or defenses).

Is there an example of a defensive power that only works for the "next set" of attacks vs. the "next attack"?

I haven't seen one, so it hasn't been an issue.
