In the SW ruleset you can use <expand/> to expand an entry in the referenceinlineindex frame, this does not appear to work in other rulesets.

Any advice?

Nope, I added it to Call Of Cthulhu and subsequent rulesets, but it isn't in the standard 3.5 etc. You can also specify an empty 'name' tag: useful in header and footer text for inline lists.
