I'm definitely a low-prep, high-improv GM and I take a lot of inspiration from images. I use OneNote for adventure design. Here is an example of how I design an encounter:

I've run this adventure twice for different groups: in one the PCs tracked them and were led into an ambush, and in the other a lone PC on watch was silently kidnapped in the night. Play to find out what happens!
Aside from that, the only other prep I do is adding the tokens, monsters and other images to FG.Last edited by Answulf; August 8th, 2017 at 06:53.

Lately, I find that the more I prepare the less my players go that route, so in the end I improvise anyways

You can, unless it's situation specific. Hence why may maps are more generic. I did put together a pretty good star-base my players raided, but they were planning that raid for a couple of sessions so I had ample time to anticipate it.
